Partnering to find practical solutions to transition in care challenges

CIHR will work together with partners to leverage our combined expertise and resources to support research that will help find practical solutions to transitions in care challenges for better patient outcomes. Together with our partners, the TiC initiative aims to improve the health and well-being of all people in Canada by supporting research that transforms the health system to optimize the outcomes of individuals experiencing transitions in care.

CIHR recognizes that there are many levers for positive change, both within and outside the health sector. To create healthier environments, it is essential to work with a wide range of partners. Together we can identify and meet shared goals and objectives.

To establish yourself as an interested researcher, partner or stakeholder for an open TiC funding opportunity, please visit the TiC Components page to access the active linkage tool. The linkage tool will enable applicants to identify interested partners to contact for the purpose of exploring potential partnership opportunities. This tool will also provide an opportunity for applicants to find other interested applicants in their research topic.

Help us advance transition in care by:

  • reducing and prevent gaps in care;
  • scaling up innovative solutions to optimize transition in care outcomes (e.g., digital health, care pathways, EMRs);
  • addressing unmet health needs of a diverse group of people; and,
  • optimizing the potential for health system impact.

Facilitating linkages

Funded partnerships must enable reciprocal flow and uptake of research knowledge between researchers, applicant partners and stakeholders - both within and beyond academia - by integrating applicant partners throughout the entire research process. Researchers, applicant partners and stakeholders will collaborate to develop the research questions, decide on methodology, collect data, interpret the findings, and disseminate the research results. This collaboration should increase the likelihood of adoption and uptake of research results.
